Paso Nuevo ICM
Paso Nuevo ICM is an Intensive Case Management (ICM) program that serves adults ages 18 and older who reside in the South Region of San Diego County and are living with serious mental illness and/or co-occurring disorders. The program supports individuals who require a higher level of care to achieve stability and increase independence, particularly those with complex needs who may be at risk of hospitalization, homelessness, or disengagement from services.
Paso Nuevo ICM utilizes a multidisciplinary, team-based, field-based approach to deliver comprehensive and individualized care in the community. Services include care coordination, support with housing and benefits, medication support, treatment engagement, and skill-building to promote recovery and long-term stability.
Services are available in English and Spanish, with a focus on serving the diverse communities of the South Region.
The program is funded by the County of San Diego, Behavioral Health Services.
Eligibility Criteria
- Adults ages 18 and older.
- Primarily serves individuals who reside in the South Region of San Diego County.
- Have a serious mental illness with functional impairments such as inability to maintain housing, employment, or education. Individuals with co-occurring substance use conditions in addition to a primary serious mental illness are welcome.
- May be experiencing homelessness or be at risk of homelessness.
- Voluntary participation.
- Individuals with Medi-Cal, those eligible for Medi-Cal, Medicare and uninsured individuals are accepted. No one is denied services based on inability to pay.
How to Refer
Referrals can be submitted on Optum’s Single Point of Access (SPOA)-ACT, FACT & Intensive Case Management website. Single Point of Access-ACT, FACT & Intensive Case Management.
If a referring party does not have access to a computer, call 1-800-842-2652 to complete the referral.
